We provide the clinical expertise, personalised planning, and ongoing support to be your best health buddy. Comprehensive Health Assessment

A head-to-toe evaluation of fall risks, medication safety, general wellbeing and home safety.

A Proactive Action Plan

Create a person-centred plan to

fix risks, maintain the strength, develop a holistic care plan that suits you the best.

Ongoing Support

Choose a plan suitable for you and our registered nurse will visit you weekly.
We liaise with your GP and specialists when things go off the rail.

Our home care services

My journey to founding SoMate Care didn’t begin in a boardroom; it began at the bedside, and in the homes of my patients.

I witnessed a consistent and troubling gap in the aging population. I saw numerous individuals begin to fear their own future after a minor fall or a new diagnosis. I saw families stretched thin by worry and getting lost with the complexity of the health system.

I realised that what was missing was not how many services available, but clarity. Not just multiple supports from different specialties but someone to connect all the dots, looking at the whole person. Not just support, but strategy. My clients didn’t need someone to simply do for them; they needed a clinical expert to partner with them – to help understand the changes in their body, to anticipate risks before they became a problem, and to create a clear, actionable plan for lasting well-being.

And so, SoMate Care was born.

The name “SoMate” reflects to our core philosophy: to be a true, compassionate companion and expert guide on your health journey. This is more than a business to me. It is a commitment to the community I serve. It is the promise that aging in place can be a chapter of life filled with confidence, grace, and vitality. We are here to make that promise a reality.
